The Ball Repair Form is the paper trail that keeps the bowling center, the pro shop, and the customer accountable to one another on every single repair. It is the document our ball repair policy is built around.
Why we use it
Ball repair is one of the easiest places for expectations to break down. The customer remembers one thing, the center staff remembers another, and the pro shop is left in the middle. This form removes the guesswork by capturing what was agreed to, in writing, signed, at drop-off — before a single tool touches the ball.
3-part carbon copy
Every form produces three copies so all parties hold the same record:
- Customer — receives the bottom copy at drop-off.
- Pro shop — keeps the pink copy once the repair is finished and signed off.
- Bowling center — retains the original for their records.
What the form captures
- Customer Section — name, phone, ball name, serial number, a written description of the damage, drop-off date, desired pick-up date/time, lane number, and the customer's signature. The signature confirms that all damage to be repaired was communicated and circled by the center staff.
- Bowling Center Employee — received-by name and initials, acknowledging the necessary repairs were clearly communicated to Virtue Bowling Supply and that all repair areas are circled.
- VBS Internal Use Only — who picked the ball up in store and when, or when it was left at the counter, plus the date the center was invoiced.
Built for clean monthly billing
At month-end, the store manager uses the completed forms to determine whether there is reason to bill the center. Because the center holds matching copies, every line on the invoice can be validated against a signed form — no disputes, no reconstructing history from memory.
Scope note
The form reinforces our standing policy: Virtue Bowling Supply determines whether a ball can be repaired, how long it will take, and what it may cost. We do not determine whether a ball should be replaced. We act as the mechanic fixing the car, not the insurance adjuster deciding if it's covered.
